If you need to reschedule your appointment, please call (563) 383-2671 at least 24 hours in advance. This allows us to provide better service for all our patients.
Nursing staff are available to answer your medical questions between 8:30 a.m. - 5:00 p.m. Please allow time for our staff to discuss your medical issues with your physician. Calls received after 4:00 p.m. may not be returned until the next business day if your physician is unavailable and your question and/or concern is non-urgent.
Prescriptions are refilled between the hours of 8:30 a.m. and 4:00 p.m., Monday thru Friday. We do not fill medications after hours (nights and weekends). Written scripts require 24 hour notice. Due to federal changes in prescription drug coverage we require copies of prescription drug ID cards.
It is your responsibility to provide accurate insurance information. Please bring your current insurance card(s) to your appointment. Failure to provide complete and accurate information may cause a delay in your claim being processed. As a courtesy we will file your health insurance claim for you. You made need to contact your health insurance carrier if you claim is unpaid after 30 days.
We will complete your medical forms upon request. Please allow 7 to 10 working days for processing. There may be a fee upon completion.
