Patients -> New Patients
Dear New Patients:
You have been scheduled to be seen in our office within the next few weeks. You will receive a new patient packet within a couple weeks that will include the forms listed below. These forms should be completed and returned before your appointment:
To provide the best medical care possible, we will request medical records from your referring doctor(s). In some cases an authorization to obtain records will be required. We will let you know if this is necessary.
Our physicians require copies of MRI or CT studies related to your neurological condition. Studies performed at Genesis facilities or Radiology Group will automatically be forwarded to our office. MRI or CT studies performed at other facilities will need to be picked up prior to your appointment. Be sure to ask for the report, in addition to copies of the films.
Please bring an up-to-date medication list to your appointment. This helps our physicians determine the right medication for you, should your condition require pharmaceutical treatment.
If you have a friend or family member assisting you with your day-to-day medical issues, it may be beneficial to bring them to the appointment with you, as they may be able to provide additional history. This information enables our physicians to make informed decisions when recommending treatment plans.
